Introduction

La Passerelle-I.D.É. is committed to promoting safe, inclusive, and culturally tailored support and services for Black and Racialized Francophones. The Case Worker plays a vital role in supporting residents through individualized case management, By incorporating culturally sensitive approach and fostering intercultural integration. This role involves developing personalized action plans, addressing systemic barriers, and connecting residents to critical resources to help them transition into independent living. Reporting to the Francophone Transition Home Manager, the Case Worker works collaboratively with staff, volunteers, and community partners to ensure residents’ success and well-being.

Key Responsibilities

Support for residents

  • Conduct comprehensive admission assessments to understand residents' needs, goals, and cultural contexts.

  • Develop and implement individual case management plans, focused on empowerment and long-term goals.

  • Provide advice and advocacy to overcome systemic barriers, including linguistic, cultural and social challenges.


Resource coordination

  • Connecting residents to services such as health care, legal aid, housing, employment and education.

  • Facilitate workshops on life skills, financial literacy, cultural integration and English language support.

  • Monitor residents' progress and adjust action plans as necessary to meet their changing needs.


Cultural advice and intercultural integration

  • Provide cultural guidance to ensure services are appropriate for residents’ unique cultural contexts.

  • Helping residents manage their transition to the wider community while preserving their cultural identity.


Collaboration and partnerships

  • Work closely with external agencies, community organisations and service providers to ensure holistic support for residents.

  • Participate in case reviews and collaborate with the francophone transition home staff to improve service delivery.


Documentation and compliance

  • Maintain detailed and confidential file notes in accordance with organizational and legal standards.

  • Prepare reports on resident progress and service outcomes for internal and external stakeholders.


Professional development and team participation

  • Attend team meetings, training sessions and community events to stay informed of best practices.

  • Participate in continuing professional development to improve skills and knowledge.


Additional Responsibilities

  • Perform additional duties and special projects as needed to support the francophone transition home operations.

Working Conditions

  • Work in a dynamic environment with residents facing complex challenges, including trauma and systemic barriers.


This position requires flexibility, including evenings, weekends, holidays, and overnight shifts, to meet the needs of residents.

Required Skills and Competencies

  • Education: Bachelor's degree in social work, psychology or related field.
  • Experience:
    •  Experience in case management, social work or working with refugee and immigrant populations.
    •  Knowledge of trauma-informed care and culturally sensitive practices.
  •  Language : Fluency in French and English (oral and written).
  •  Skills:
    •  Strong organizational, advocacy and interpersonal skills.
    •  Ability to handle sensitive situations with empathy and professionalism.
    •  Knowledge of local resources and services for refugees and vulnerable populations.
